DI 28057.010 Expedited Reinstatement (EXR) Case Receipt

A. Procedure – EXR Case Identification

Identify expedited reinstatement (EXR) cases by the flag placed on top of the folder. The flag will state, “Expedited Reinstatement Case – Priority Handling” (see Exhibit 4 DI 13050.105C for an example). The EXR package should include the following.

  1. 1. 

    Expedited Reinstatement Request form(s) (see exhibits in DI 13050.105):

    • SSA-371, Request for Reinstatement for title II;

    • SSA-372, Request for Reinstatement for title XVI Individual or Disabled Spouse with Work Activity; and/or

  2. 2. 

    The appropriate title II/title XVI paper application (i.e., a SSA-16-F6, a SSA-10-BK, or a SSA-4-BK, SSA-8000BK). The heading of the form will have been changed by the FO to read as a supplement to the request for reinstatement.

  3. 3. 

    SSA-454-BK, Report of Continuing Disability Interview.

  4. 4. 

    SSA-827s, Authorization for Source to Release Information to the Social Security Administration.

  5. 5. 

    The comparison point decision (CPD) file or documentation of the efforts made to locate the file if the file is not included. Refer to DI 28035.000 for lost folder procedures.

  6. 6. 

    SSA-820 or SSA-821 and SSA-823.

    • The month and year of the EXR request and the SGA determination is recorded on the SSA-823.

The FO should provide other information on the flag that could affect the case processing (possible dual actions, folder association issues, etc.).

NOTE: EXR cases are excluded from prototype procedures, as are initial claims that may be associated with an EXR case.

B. Procedure - Initial Case Receipt

1. Case receipt

Receipt the case as a continuing disability review (CDR) by entering the following information:

Obtain data from the original SSA-831-U3 (Disability Determination and Transmittal Form). Update with information provided by the FO or a query.

Enter “N”

Enter the office code of the office that sent the folder

CDT

Enter a CDT code of 05

2. Multiple actions pending

If the EXR action is received simultaneously with an initial claim action on the same individual, for the same benefit, follow locally established receipt and clearance procedures. Depending on variations in adjudicative protocols and system limitations that may preclude controlling multiple actions on the same title and SSN, establish manual control processes as needed. It may be possible to sequentially establish the actions if adjudicative timeframes vary (for example, receipt the EXR action, complete that action in the system, then receipt the initial claim action and complete the initial claim). See DI 28057.045.

3. Transfer for development

When sending a case to the FO for development, treat the case as a transfer case and input the AH, AN, DEC, DST, SCD, SO and TYP fields. The DEC is “TR” and the SO is “C”. When the case is returned from the FO, receipt it again, using the procedures in DI 28057.010B.1. Do not use return (RTN) input procedures when receiving a case back from the FO after development.
