DI 39506.270 Ordering Financial Management Forms

Procedure

The Office of Disability Determinations (ODD), Division of DDS Resource and Workload Management (DDRWM) will release the electronic financial management forms via email to the regional offices. The State DDS's can obtain the electronic forms from their regional office. The forms will also be available electronically via the ODD DDRWM Intranet website.

The financial management forms include:

    SSA-871, State Agency Schedule of Equipment Purchased for SSA Disability Programs
