Maintenance of Time Records
Charges for part-time work in conjunction with disability determinations made by a DDS will be supported by time records maintained by the agency.
Record the number of part-time hours as well as hours worked by full-time employees on Form SSA-4514.
Send this form to the SSA RO and SSA CO each quarter.
