SSA has agreed to obtain supplemental health insurance information, to assist the State in determining whether any third party medical insurance exists, for the individual or the individual's family which can be used to defray the costs of medical care - MSSICS screen BTPL or SSA-8019-U2. Refer to POMS SI 01730.045. Any SSA-8019-U2s should be mailed to:
