GN 00302.790 Request for Search of Deceased Person's Record

A. Who must sign request

The Bureau of the Census will honor a request for a census record for a deceased person only if the SSA-1535-U3 or BC-600 is signed by one of the following:

  • A blood relative in the immediate family (i.e., parent, brother, sister or child), or

  • The surviving wife or husband, or

  • The administrator or executor of the estate, or

  • A beneficiary by will, intestacy, or insurance.

B. Evidence required

Relationship of Person Requesting Deceased's Record Evidence Required by Bureau of the Census

Legal representative

  • Certified copy of the court order naming the legal representative

  • Death certificate

Beneficiary by

  • will, or

  • intestacy, or

  • insurance

  • Legal evidence of beneficiary status

  • Death certificate

All others

Death certificate

C. Note

The Bureau of the Census will accept a photocopy of any document submitted with the request form in accordance with this instruction.
