If the event for which the document(s) is being requested occurred on an island, show the name of the town and the island. Include the name of the parish (Freguesia) in which the event occurred.

Office of the Civil Registrar (Conservatoria do Registo Civil) in the town in which the event occurred or the nearest town.

a. Divorce by Mutual Agreement (after 1995)

Civil registry office at which the divorce was obtained.

b. Other

Judicial officer of the court in which the decree was obtained.
