GN 00904.075 Requesting Information on When Wages were Earned from Employers in Other Districts

Use Form SSA-L725 (Letter to Employer Requesting Him to Furnish Wages Earned by Beneficiary) to reconcile material discrepancies between wages earned and wages paid in deduction cases. The information requested by Form SSA-L725 is necessary in postadjudicative earnings investigations when the beneficiary is unable to secure information regarding his/her earnings or is unable to support his/her allegations of wages earned. In some instances, completion of the form may be necessary during the development of an initial claim when the imposition of deductions is pertinent or when there is reason to question the allegations.

Send SSA-L725 directly to an employer outside your district following instructions in GN 00904.070 and see GN 00904.082, Exceptions to Use of Form SSA-562-U3. (The use of this form is not restricted to lag earning information.) Make the initial request with Form SSA-562-U3 through the servicing office if special wage payments are involved or incorrect reporting is likely. Send a Form SSA-562-U3 to the servicing office if you do not receive a reply to an initial direct request, informing the servicing office that a previous direct mail request was unproductive, or if the reply requires additional information or clarification. Attach copies of requests and replies to aid the servicing office in its contact.
