GN 00904.100 Attachments to Form SSA-562-U3

In routine cases, the requesting office need not prepare partially completed letters or forms for use by the assisting office. When the requested action is unusual or complicated, provide the assisting office with copies of background material, forms, etc., which will aid them in completing the requested action. For example:

  1. A. 

    A copy of a PC request for additional development requiring the assistance of another DO should be attached to the SSA-562-U3.

  2. B. 

    A request for contact with an employer on a coverage question should be accompanied by the coverage questionnaire completed by the WE or surviving claimant.

  3. C. 

    A request for medical evidence should be accompanied by the appropriate authorization for its release. The requesting office will fill in the identifying information blocks on the appropriate medical report form and attach it to the SSA-562-U3. In addition, the alleged date of onset should be included in the appropriate block of Form SSA-562-U3.

    Both the requesting office and the assisting office will use the SSA-562-U3 as a cover sheet for transmitting documents involved in the request.
