DI 11005.037 Applicant Forms Needed in Concurrent Claims

Policy

When an applicant files concurrent claims, only one disability report needs to be completed. In the situation in which adult concurrent claims are filed, the SSA-3368 should be completed. In the situation in which a title XVI disability or blindness claim is filed for a child (i.e., an individual who is under age 18) concurrently with a title II disability claim (which would normally require completion of adult forms), the FO is responsible for obtaining an SSA-3820 and an SSA-3379.
