DI 22505.024 Developing Evidence from the Office of Personnel Management (OPM)

A. Process

1. General

OPM will furnish medical evidence on individuals who have filed for civil service disability retirement.

2. Record Content

These records may include:

  • A letter from the treating source(s)

  • A Certificate of Medical Examination (with detailed clinical signs and laboratory findings)

  • A report from the employing agency describing job duties and the employee's ability to perform those duties.

B. Procedure

Follow the instructions below to request the evidence.

1. General

  1. a. 

    Send the request to:

    Office of Personnel Management

    Employee Service and Records Center

    Claims Correspondence Section

    Boyers, Pennsylvania 16017
  2. b. 

    Include a signed SSA-827.

  3. c. 

    Include the claimant's:

    • Name

    • Current address

    • Date of birth

    • Civil service claim number, or, when not immediately available, last government employer, location, dates of employment, and date of retirement.

2. Followup

If a response is not received within 45 calendar days from the date of the initial request, send a followup request.
