SI 00501.419 Blind or Disabled Children of Military Personnel Stationed Overseas - Exhibits

A. Exhibit 1 - Modernized Development Worksheet (MDW) to Cumberland

  1. 1. 

    Send an MDW to the Cumberland FO (See MSOM MDW 001.002 - Instructions on Accessing the MDW).

  2. 2. 

    Complete the entire MDW and use the following information when completing the following fields.

    • Show “275” as the FO;

    • Show “DC” as the ID;

    • Complete applicant's contact information (e.g., name, foreign address and phone number, etc.);

    • If reporting a COA, in MISC show “Attention Military Children Overseas Coordinator”, date expected to join military parent overseas MM/DD/YY, and date case file mailed MM/DD/YY;

    • If reporting an Inquiry or Appeal Request, in MISC show “Attention Military Children Overseas Coordinator;

    • Show “MIL CHILD” as the ISSUE;

    • Show “T16CG” as the CATEGORY;

    • Document REMARKS with any available information in SI 00501.416B.1.;

    • If a protective writing is received, show the protective filing receipt date in REMARKS; and

    • If reporting an Appeal Request, show the date the Appeal Request was received.
