SI 00604.045 Completing Part II (Living Arrangements) of the SSA-8000-BK: General

A. Time period for questions and answers

We designed the questions in Part II to document living arrangements as of the day the claimant signs the form. Any changes in living arrangements since the first moment of the filing date month should be recorded in question 39 and explained in Remarks.

1. Additional information needed

If there is a need to document a change in living arrangements that occurs after the claimant files the claim and before adjudication, complete SSA-8006-F4 (Statement of Living Arrangements, In-Kind Support and Maintenance) or the SSA-8007 (Statement of Living Arrangements). For instructions on completing the SSA-8006-F4 or SSA-8007, see SI 00835.600.
