Authorization to Collect A Fee
If your organization is seeking authorization to collect a fee for representative
payee services, follow the enclosed instructions carefully. Send form SSA-445, Application
to Collect a Fee for Payee Services, along with the requested documents to the address
shown above. Please make sure that your application contains all the necessary information,
as shown on the enclosed pages.
After we receive your application, we will respond within 30 days. You are not permitted
to collect a fee unless we notify you in writing that you have been authorized to
do so.
