The following section reflects the guide paragraphs to be used and the suggested captions for use with those guide paragraphs. They are arranged in the order they should appear in a notice. If more than one paragraph is used, show them in the order that they appear below.

The term “null” in a fill in means that the options for the fill in are those listed or a blank.

ERA901 — REPLY TO LETTER

We are writing to reply to your letter dated (1) . You said that (2) . (3)

ERA902 — REFERRAL TO ANOTHER AGENCY

ERA903 — RETURN NOTICE

We are returning the Potential Private Pension Benefit Information notice that you sent to us.

 

ERA904 — RETURN NOTICE — NO PENSION INFORMATION

We are returning your request for information about deferred vested pension rights because we have no pension information recorded under the name and Social Security number you furnished.

 

ERA911 — MAINTAINING PENSION INFORMATION

The Social Security Administration maintains pension information about individuals who have left work covered by a private pension plan but who have earned a right to pension benefits under the plan. We give this information to these individuals (or their dependents or survivors) upon request or when they apply for Social Security benefits.

 

ERA912 — POTENTIAL ENTITLEMENT

We sent the Potential Private Pension Benefit Information notice to tell you that the (1) pension plan had reported that (2) may (3) entitled to benefits from the plan. (4)

This information was reported on Form 5500, Annual Return/Report of Employee Benefit Plan - Schedule SSA, Annual Registration Statement Identifying Separated Participants with Deferred Vested Benefits.

 

ERA931 — DEFERRED VESTED BENEFITS

When a person leaves a job covered by a private pension plan before retirement age, but has earned a right to future pension benefits under the plan, such benefits are called “deferred vested benefits”.

 

ERA932 — UNTIMELY REPORTS

We do not always receive and process pension reports in the same year to which they apply.

 

ERA933 — YEAR REPORTED

The “Year Reported” column heading refers to the actual year for which the Plan Administrator reported this information.

 

ERA934 — NO DECISION MADE

The code M for “other” shown under Type of Annuity and/or Payment Frequency means that no decision had been made at the time the other pension information was provided.

 

ERA935 — VALUE OF ACCOUNT

The amount shown under Value of Account is the value of the account when (1) left the job covered by the pension plan.

ERA944 — PRIVATE PENSION BENEFITS

This information concerns private pension benefits only. It does not apply to your Social Security earnings record nor any Social Security benefits to which you may be entitled.

 

ERA950 — IF YOU WANT TO APPLY FOR BENEFITS

Please contact the Plan Administrator listed in the notice to find out how to apply for pension benefits under the plan or for any additional information.

 

ERA955 — IF THE PLAN ADMINISTRATOR ADDRESS IS WRONG

The address we gave you for the Plan Administrator is the only one we have. Since that address is not correct, please contact your former employer for the current address. If you cannot locate your former employer, check the reference department of your local library or contact your State's corporation commission, office or division. This agency should be listed in the blue pages of the telephone directory under State government. It is often a part of the Secretary of State, Commerce or Taxation Departments.

 

ERA960 — PAYMENT UNDER THE PLAN

We can give you only the information given to us by the Plan Administrator. We do not check the accuracy of the information. We also realize that the actual method and/or amount of payment under a plan may differ from that shown on the Potential Private Pension Benefit Information notice. The reasons for this include:

  • changes to plan rules;

  • benefit options selected; and

  • mistakes in the information given to us by the Plan Administrator.

     

ERA965 — IF YOU HAVE A COMPLAINT

If you have a complaint about (1) , contact the local office of the Department of Labor's Pension and Welfare Benefits Administration. If this office is not listed in your telephone directory, write to the Department of Labor, Pension and Welfare Benefits Programs, Room 4659, 3rd Street and Constitution Avenue, N.W., Washington, D.C. 20216.

ERA970 — WHAT YOU NEED TO DO

We cannot give out the information from a person's record without that person's signed request. Please complete, sign and return your request and we will consider it further.
