GN 01701.145 Entitlement to Supplementary Medical Insurance (SMI) in Totalization Claims

A. Policy

Totalization beneficiaries not entitled to premium-free hospital insurance (HI) (see GN 01701.140) must meet certain residence and citizenship or alien status requirements in order to qualify for SMI.

NOTE: Since a beneficiary must be entitled to premium-free HI or be age 65 to qualify for SMI, Totalization beneficiaries cannot be entitled to SMI until age 65 unless they are entitled to premium-free HI on a different Social Security number (SSN).

B. Procedure

When a Totalization beneficiary not entitled to premium-free HI wishes to file for SMI, take and process through the modernized claims system (MCS) an uninsured Medicare claim. As an alternative, obtain and process a CMS-18F (Application for Hospital Insurance), CMS-4040 (Request for Enrollment in Supplementary Medical Insurance), or a statement providing similar information.
