GN 01718.010 How the Foreign Service Post (FSP) Develops and Processes Applications - Procedure

When a claimant contacts the U.S. Embassy in Stockholm to apply for U.S. benefits, obtain the following applications.

  Form SSA-2490 F4, Application for Benefits Under a U.S. International Agreement (GN 01701.110).

• •

  Regular RSDI applications if the applicant is filing for U.S. benefits.

Request the Swedish coverage record from the SLAFU if there is potential entitlement to U.S. totalization benefits. Make the request on form SSA-2960-U3-SW.

Refer the following to OIO.

• •

  All claims for U.S. benefits.

• •

  A copy of Form SSA-2960-U3-SW if a coverage record was requested from Sweden.

Generally, refer individuals who want to file claims for Swedish benefits to the Swedish local office.
