Claims packages are prepared to request Spanish coverage records and to inform the Spanish agencies about claims for Spanish benefits. (The application and evidence required for the packages are described in GN 01721.205 - GN 01721.215 and GN 01722.006).
Use these instructions for preparing claims packages.
Prepare a claims package only if a Spanish coverage record is needed. Include the following:
Liaison form SSA-2960-U3-SP.
Form E/USA 2 (Statement of Working Life).
Spanish Navigation Book (if it was submitted with form E/USA 2).
Prepare a claims package that includes:
U.S. totalization earnings record
NOTE: Enter “form USA/E4” in the upper right hand corner of the earnings record.
Form E/USA (Spanish Application Form)
Spanish Navigation Book (if it was submitted with form E/USA).
Medical evidence from OIO files if the Spanish claim is for disability benefits.
Any additional evidence that the claimant has submitted for the Spanish claim.
