In addition to Form SSA-2490-F4, an applicant for Greek benefits may need to complete one or more special Greek application forms. The form(s) to be completed vary depending on the type of benefit claimed.

The special application forms are not taken in the FO. After receiving a claim for Greek benefits that was filed with SSA, the Greek agency will mail the appropriate Greek applications, if necessary, to the applicant with instructions requesting that he or she complete the forms and mail them directly to the Greek agency with any necessary additional evidence.

If an applicant brings a Greek application form to the FO:

  provide any necessary assistance in completing the form, and

• •

  return the application(s) to the applicant and advise him or her to return it directly to Greece at the address shown on the application form.
