Policy for forwarding certified coverage records

The Brazilian certified coverage record is the Form BR/USA 6 (Certificate Concerning the History of Insurance in Brazil). It is the only acceptable form for the certification of the Brazilian coverage record. Forward certified coverage records in any other format to the Division of Training and Program Support for approval.
