GN 03316.070 Disclosure Without Consent to Employment Networks (EN) to Assist in Administering
the Ticket-to-Work Program

An EN is a qualified entity that enters into an agreement with us to function as an
EN under the Ticket-to-Work program.

An EN is responsible for coordinating and delivering employment, vocational rehabilitation,
or other support services to beneficiaries with disabilities who were assigned a ticket
to the EN.

An EN delivers services under an Employment Plan.

We may disclose information to ENs who have an approved business arrangement with
us to perform vocational rehabilitation services for disability beneficiaries and
recipients, in connection with the Ticket-to-Work program under the Social Security
Act Section 1148.
We may disclose when the EN is a State agency without consent under the health and
income maintenance Privacy Act routine use exception, see GN 03314.005.
Otherwise, the EN must submit a Form SSA-3288, Consent for Release of Information,
signed by the beneficiary, before we can disclose information to the EN. We may disclose
relevant information from the following systems of records, including but not limited
to:

60-0050 Completed Determination Record – Continuing Disability;

60-0295 Ticket-to-Work and Self-Sufficiency Program Payment Database; and

•
60-0300 Ticket-to-Work Program Manager (PM) Management Information System.
