If it becomes necessary for the patient to transfer to a different physician or HHA
(in a different locality) after the timely establishment of the required physician's
plan, the original plan may be continued in the new locality if:

There is a referral by the patient's physician in the old locality of both the patient
and the plan to a physician in the new locality.

The patient's physician in the new locality accepts the original plan of treatment
and assumes the responsibility of conducting the required periodic reviews of the
plan. The plan could be modified from time-to-time as determined necessary by the
patient's physician in the new locality.
