HI 00825.055 Undeliverable HI/SMI Letters and Forms

The Health Insurance and Inquiries Examiner is responsible for processing undeliverable items such as HI/SMI notices. Forms CMS-40, CMS-40B, and health insurance cards. Obtain the claims folders for these undeliverable items and process as indicated below. Undeliverable premium notices will be examined daily by the Records Analysis Clerk and handled as in HI 00825.070.
