Five major problems may arise in the system:
An error (or omission) in the MBR created the same error in the HI master tape. Errors are caused by wrong information in the claim file, “punching errors,” etc.
An exchange problem existed and data on the MBR was not completely or correctly transferred to the HI master tape.
An exchange problem existed and data on the HI master tape was not completely or correctly transferred to the CWF Master File.
A problem with the HI master tape system itself created an error only on the HI record.
An intermediary or carrier's internal problem. This is indicated when correct information exists on the MBR, the CWF, and the HI record.
