Use the PF20 key to create a notice and next to UTI, enter E3474R. Enter an “e” in the head command line and enter all prompt screen information. Cursor down to
#SIGNATURE and add a line (i.e., enter an “a” on the line command). Cursor down to the new blank line and type .PAG. This will
cause a page break so that when the special reconsideration model command is entered,
it will begin at the top of the page after the beneficiary notice. Cursor down to
.PAG and add as many blank lines that are needed to type in the special model for
the Form SSA-662 and all required local reconsideration paragraphs. Cursor down to
the new blank lines and type #PCRCN0662 (0 = zero). After this model call, type in
all required UTIs.
EXAMPLE: The Form SSA-662 requires the following UTIs - M30, PSE3, PSE2 and M30. On the line
command for .PAG, type a5. This will give 5 blank lines where you will type:

NOTE: If a signature is required on the Form SSA-662, then you will also have to add #SIGNATURE.
