The CE provider will describe and discuss, as appropriate:
1. Appearance, behavior, and speech (if not already described). For example, dress,
grooming, and appearance of invalidism;
2. Thought process. For example, loosening of associations;
3. Thought content. For example, delusions;
4. Perceptual abnormalities. For example, hallucinations;
5. Mood and affect. For example, depression or mania;
6. Sensorium and cognition. For example, orientation, concentration, remote memory,
recall of new information, fund of information, and estimated intelligence;
7. Judgment and insight;
The description of the claimant’s mental status must not be an enumeration of the
symptoms reported by the claimant (or other source) in DI 22510.112 C.2., rather the description must be the examining source’s description of the above items.
8. Further, the CE provider will describe and discuss general observations, as appropriate;
9. How the claimant came to the examination;

a. Alone or accompanied;

b. Distance and mode of transportation; and

c. If by automobile, who drove.
10. Attitude and degree of cooperation;
11. Posture and gait; and
12. Involuntary movements.
