Certified copies cannot be obtained without charge if hardship exists. If the document
                  is requested by the PSC, OIO or is needed to resolve a death alert, the parallel FO
                  is authorized to pay for the document with imprest funds.
               
               Requests made to the Utah BVS for birth or death documents must be accompanied by
                  an enlarged photocopy of the front and back of an acceptable identification document
                  from the requestor and a signed statement.
               
               All requests for vital records should be made directly to the Utah Department of Health,
                  Office of Vital Records and Statistics accompanied by the appropriate fees. Any requests
                  that are faxed/mailed to FO 900 will be returned to sender. Current response time
                  to obtain a certified copy of the record will vary from 2-6 weeks.
               
               Certification of divorce or marriage is available through the State Vital Records
                  office for the years 1978-present.
