A. Use of Form

This form will be used primarily by the offices which serve Europe, Asia, Africa and Central and South America and those offices serving residents of Canada and Mexico. It requests evidence of age, identity, and U.S. citizenship from U.S.-born applicants for SSN's whose address is outside the 50 States, District of Columbia, Puerto Rico, Guam, the U.S. Virgin Islands, Swain's Island or American Samoa. (See RM 10220.015.)

B. Preparation of Form

  1. 1. 

    Enter DO address.

  2. 2. 

    Enter applicant's mailing address.

  3. 3. 

    Check the appropriate block.

C. Other Actions

  1. 1. 

    If SS-5 has already been received, return it with the letter. The applicant should then resubmit the SS-5 and evidence together.

  2. 2. 

    If the letter is being sent in reply to a request for an SSN application form, enclose a blank SS-5.
