  A spouse, surviving spouse, or parent should file an SSA-760-F4 (Certificate of Support), within the specified time limit whether or not a claim is filed.

An SSA-3 (Spouse's Certification) must accompany the spouse's SSA-760-F4.

If the support determination is favorable, evidence supporting all allegations is required.

NOTE: This form is not used in child dependency cases.
