DI 28084.035 Diagnosis Codes (Items 21 and 22)

A. Introduction to completing Items 21 and 22

For instructions to complete Items 21 and 22 when processing a continuing disability review (CDR) on the Forms SSA-832 (Cessation or Continuance of Disability or Blindness Determination and Transmittal –Title XVI) or SSA-833 (Cessation or Continuance of Disability or Blindness Determination and Transmittal – Title II). Follow the procedures for Completing Item 16A and 16B (Primary and Secondary Diagnosis, Body System Code and Impairment Code) on the SSA-831 Disability Determination and Transmittal in section DI 26510.015.

B. Special situations

Use the specific coding instructions for a CDR on the following situations:

  1. a. 

    Do not use code 7649 (Low Birth Weight 1200 to 2000 Grams) or code 7650 (Low Birth Weight to Weight Under 1200 Grams) for a continuance determination for a low birth weight CDR. Low birth weight is not a medically determinable impairment (MDI) after the attainment of age one; and

  2. b. 

    Do not use code 0000 (None Established – No Medical Evidence in File) as the primary code in Item 21. This code is a system limitation for posting CDR determinations to the Master Beneficiary Record (MBR).
