The purpose of this area is to allow space for entering the date SSA actually receives a completed and signed application. This date is the receipt date. See GN 00204.007B for situations when the receipt date may be earlier than the actual date the signed application is received by SSA.

Write in or date stamp the actual date (month, day, and year) that SSA receives a completed and signed application.
