SI NY01801.006 (New Jersey) SSA's Responsibility For Providing Verification

A. General

The FS application and all available FS documentation should be forwarded to the servicing FS agency, within one workday after the day the FS application is taken. The FO will mail the material using the SSA-4233 "Social Security Administration Transmittal For Food Stamp Applications". SSA interviewers will also advise the applicants that they will have to provide whatever documentation is needed by the FS office. The SSA interviewer should advise the applicant what proofs, if any, are still required by the FS office. The interviewer should stress that these should be mailed directly to the FS office and that the proofs required by SSA must be submitted separately. The SSA interviewer will give the applicant a self-addressed envelope in which the verification required for FS should be mailed directly to the FS office.

B. Mailing the application

FOs should mail the completed forms to their local FS office.

C. Ordering new applications

Additional applications may be secured by contacting your local county welfare agency.
