HI 01015.030 Recovery of Carrier Overpayments

In addition to premium deductions or collections, there may be occasions when the program service centers will be requested to recover a carrier overpayment from a beneficiary's monthly social security check. The Benefit Authorizer will prepare the Form SSA-666 charging the FOASI or FDI trust fund and crediting the FSMI or FHI collection account, as appropriate. If, after making such a deduction, the beneficiary objects, and repayment to the beneficiary is made, a Form SSA-666 must be prepared to charge the FSMI or FHI collection account and credit the FOASI or FDI trust fund.

In order to assure proper trust fund accounting, any SSA-666 prepared, charging or crediting 75-20 ×8005, must contain an explanation in the remarks indicating that the SSA-666 is being prepared because of a “Carrier Overpayment” deduction or waiver.
