B. REQUESTING INSTRUCTIONS

  • If a prescribed application form is not filed within 30 days after receipt of the notice of death, the 6-month notice will be mailed to the survivor shown on Form SSA-721. Where no application is filed and the identity of such survivor is unknown, mail the 6-month notice to the deceased's last known address. Rather than use the deceased's name by itself in the address, refer to the estate (e.g., The Estate of John Doe). Show all information for this notice on SSA-573.
