Obtain answers in Part V for the time period specified in the questions .

NOTE: Several States have Combined Application Projects (CAP) or supplemental nutrition assistance program (SNAP) waiver projects. In these States, the SSI application becomes the SNAP application for some applicants. Refer to regional instructions if your State has a SNAP waiver project.
