GN 00308.038 Louisiana Vital Records

 

A. Process

The Louisiana Vital Records Registry in New Orleans is the repository for all Louisiana birth certificates less than 101 years old and all death certificates less than 51 years old. However, a recent change now allows for issuance of birth certificates at the parish level by clerks of court in most parish courthouses for an additional surcharge. Check with the local clerk of court or the State website to determine if a parish is participating in this new service.

c. Divorce Records and Marriage Records other than for City of New Orleans

The Louisiana Vital Records Registry can verify most divorce and marriage records. If the record is not on the State index, however, or GN 00305.020 or GN 00305.140B. requires a certified copy of a divorce or marriage record, send the request and the appropriate fee to the appropriate parish clerk of the court.

2. Cost of Copy:

Full Birth Certificate $15.00

Short form birth card $ 9.00

(Birth certificates at local parish courthouse may have an additional surcharge)

Death $7.00

Marriage $5.00

Flat fee of $5.00 for death or marriage record from State Archives

NOTE: As of 10/01/2003, an additional State charge of $.50 per transaction must be added to the above fees.

3. Type of Payment:

For a certified record, make the money order or check payable to Vital Records if you send the request to the Vital Records Registry in New Orleans. Make the money order or check payable to Secretary of State if you send the request to the State Archives address, and make the money order or check payable to the specific parish clerk of the court if you send the request to a parish (e.g., “Jefferson Parish Clerk of Court”).

4. Remarks:

Can FO 800 verify birth/death for entire state and Orleans parish marriage record? Yes. Consent and ID are not required. For birth records, use EVVE (GN 00302.980). If not successful, send e562 and be sure to click on "Help" to supply all necessary information including the BC number.

Can FO 803 verify State archive record? Yes. Consent and ID are not required..

Can FO obtain free certified copy or photocopy of birth/death/marriage record? No. If the claimant meets the criteria in GN 00301.210 for SSA purchase of certified evidence, prepare a Third Party Payment using your FO CAN. On all requests you send directly to the State for a certified copy of a birth or death certificate, include a copy of a picture ID for the registrant and a signed consent form giving permission for the document’s release to SSA. Please note that the State office does not provide certified copies of divorce and marriage records for other parishes, only for City of New Orleans. Send requests for certified divorce or marriage records outside the City of New Orleans to the Clerk of the Court in the appropriate parish office.
