A. Use of Form

The SSA-2038-F3 may be used whenever a DB determination is required (see GN 00302.405). The form is designed to help guide the adjudicator in determining the DB and providing explanations when required by POMS instructions (see GN 00302.170).

Where fewer details and less explanation are required (e.g., in cases where no material discrepancy exists), only part of the SSA-2038-F3 need be completed, as instructed on the form itself.
