SI 02305.117 Informing Recipients of their Reporting Responsibilities - Redeterminations (RZs)

A. Introduction

This section provides procedures on informing recipients of their reporting responsibilities.

B. Policy

Recipients are required to report changes in their circumstances that may affect their eligibility for and the amount of their SSI payments.

SSA personnel discuss reporting responsibilities during personal contact interviews. Written instructions are provided for RZs mailed by the FO.

SSA provides written instructions on several paper forms and SSI Claims system printouts.

C. Procedure

1. Face-To-Face Interviews

Review recipient reporting responsibilities and provide the recipient with a tear-off sheet at the end of a face-to-face interview, as follows:

    When using paper forms detach the tear-off sheet and print the recipient's name/SSN and the date on the tear-off sheet. The SSI Claims system prints this information on the first page of the reporting responsibilities pages of the RZ printout.

    Review the reporting responsibilities. See instructions in SI 02301.005D.

  • •

    When using paper forms print the FO number (or “800” number) and the FO address on the tear-off sheet. The SSI Claims system prints this information on the first page of the reporting responsibilities pages of the RZ printout.

    Explain how the recipient may report changes.

  • •

    Give the recipient the tear-off sheet from the paper forms or the SSI Claims system reporting responsibilities pages of the RZ printout.

2. Telephone Interviews

Review recipient reporting responsibilities and provide the recipient with a tear-off sheet after a telephone interview, as follows:

    When using paper forms detach the tear-off sheet and print the recipient's name/SSN and the date on the tear-off sheet. The SSI Claims system prints this information on the first page of the reporting responsibilities pages of the RZ printout.

    Review the reporting responsibilities. See instructions in SI 02301.005D.

    When using paper forms print the FO number (or “800” number) and the FO address on the tear-off sheet. The SSI Claims system prints this information on the first page of the reporting responsibilities pages of the RZ printout.

    Explain how the recipient may report changes.

  • •

    Mail the tear-off sheet with the SSI Claims system RZ printout or SSA-8203-BK when they are sent to recipients for signature. Include a brief covering note explaining what the tear-off sheet is.

    EXAMPLE: You are responsible for reporting changes in your circumstances that may affect the amount of your SSI payment. The enclosed form lists the things you have to report. If any of these things change, call or write your Social Security office. The address and phone number are written on the form.
