The ND Division of Vital Records (DVR) requires proof of identity for an individual
requesting a birth or death record. Acceptable forms of identification are listed
on the ND DVR site. An individual authorizing someone else to receive a certified
copy of their birth or death record must complete the form “Authorization to Receive
a Certified Copy,” which is also available on the ND DVR site under "Forms".
Certified copies of divorce records can only be obtained from the county clerk or
recorder in the county in which the divorce or annulment was decreed.
Effective, January 1, 2008, all certified copies of marriage records should be obtained
from the county where the license was originally purchased and filed.
NOTE: Fees for certified copies of marriage records may vary from county to county.
Requests should be made directly through the DVR. Requests may be made by a visit
to the office, mail, or the secured Internet application form. The DVR does not accept
telephone requests.
Requests for birth certificates for illegitimate children must be accompanied by a
consent statement signed by the subject individual (if 18 or over) or by the parent
or guardian.
