Sweden uses Form SW/US 2 to:

    transmit claims and related material to OIO,

    send requested information to OIO,

  • •

    request information from OIO.

B. Procedure

Follow these steps when Sweden requests information.

    If no claim has been filed under the agreement, obtain a consent statement if none is in file.

    Obtain the requested information.

  • •

    Complete Form SSA-2960-U3-SW, showing the information that was requested.
