Use the appropriate impairment code from the table below if the special senses or speech impairment does not meet or equal a listing:

  Use 3694 only if the visual impairment meets listing 2.02, 102.02, 2.03A, or 102.03A.

  Use 3694 (Statutory Blindness) as primary diagnosis when the established disability onset date (Item 15 of the SSA-831) is the same as the date statutory blindness begins.

  Use 3694 (Statutory Blindness) as the secondary diagnosis when the established disability onset date (Item 15 of the SSA-831) is based on another impairment and that impairment's onset date is before the onset date for statutory blindness.
