RM 01301.500 Requests for FO and TSC Assistance in Processing Requests for Statements With Questionable Social Security Numbers or Identifying Information

This section discusses field office (FO) and teleservice center (TSC) procedures for assisting the public and WBDOC in handling requests for Social Security Statements, Form SSA-7005-SM-OR, that contain:
no SSN or an incomplete, impossible or illegible SSN,
an SSN or identifying information that does not match SSA records, or
other factors that result in issuance of a Form SSA-7014 to the requester.
The problems may surface while WBDOC is reviewing and keying request Forms SSA-7004-SM-OP1, or the Statement 2000 system may identify them and send a SSA-7014 letter to the requester, asking for corrected information.
B. Kinds of requests
The kinds of requests for assistance that are covered in these instructions include:
walk-in, telephone or written inquiries to the FO or TSC from SSN holders who requested Statements and have questions about SSA"s response asking for corrected SSNs and/or identifying information,
Forms SSA-7004-SM-OP1 and SSA-7014 forwarded by WBDOC for FO help in getting the correct SSN and/or identifying information from the requester, and
requests from WBDOC for FO action to update or correct SSA records to reflect information provided by the requester.
