RM 03254.003 Examine Plan Participant's Social Security Number

1. A. 

   To obtain information from the plan participant's deferred vested rights pension record, we need a complete, legible, and correct social security number.

2. B. 

   Examine each request received to determine if the social security number is acceptable and legible.

   1. 1. 

      No SSN, or an Illegible, Incomplete, or Impossible SSN.

      1. a. 

         Response is to be released to the plan participant.

         1. 1. 

            Return the request with a Form SSA-L7038 Social Security Administration Earnings Information, check the proper box.

         2. 2. 

            Address envelope for mailing.

      2. b. 

         Request is to be released to a third party.

         1. 1. 

            Return the request with a Form SSA-L7038, check the box noted: “The Social Security number is not shown on the request, or is incorrect. Please copy the number as it appears on your Social Security card. If you do not know the number, or cannot find it, please provide sufficient identifying information to help us locate your number." (See the back of this form for requirements) ”

      Acceptable SSN — Continue processing according to RM 03254.004.
