RM 03256.003 Search and Locate on Microfilm, the Schedule SSA Corresponding to the Exceptions (4404)

A. Receive form SSA-L99-C1 with exception conditions

  1. 1. 

    Exceptions will be in DCN order.

  2. 2. 

    The following information may be missing from the exceptions.

    1. a. 

      Plan Name

    2. b. 

      Plan Number

    3. c. 

      Plan Administrator (name and address) or

    4. d. 

      Identification Number (Plan Administrator)

B. Obtain microfilm to assess exceptions

Obtain Deferred Benefits Microfilm corresponding to the first five digits (Year and Pension Block Number) of the DCN as shown in the lower left corner of the form SSA-L99-C1.

  1. 1. 

    Locate the item on film by using the last five digits of the DCN (Unit and Sequence Number), and the social security number and name as shown on the SSA-L99-C1.

  2. 2. 

    Conduct the microfilm search in accordance with the following instructions:

    • Check at least five pages before and after the position in which the page should have appeared.

    • Check the corrections at the beginning of the reel of film.

Pension items which have a sequence number of “000” will be found in corrections at the beginning of the reel of film prior to 1980 processing.

NOTE: Due to an operations problem, it was necessary to refilm blocks 001-011 for processing year 1976. Therefore, many of the corrections for these blocks ONLY will be found in each unit immediately following the Advice of Transmittal.
