RM 03803.059 Followup on Field Office Development

A. First Followup (Pull Date Shown in Block 1 of SSA-6562 Comes Due)

  1. Pull out the first copy of the SSA-5533 U4. It is not necessary to attach file material.

  2. Across the top of the form, in red, write “First Request for Status.”

  3. Forward the followup form to the field office involved.

  4. Cross out pull date in block 1 of SSA-6562. Enter a new pull date in block 2 which is 30 calendar days from the date the followup is sent.

  5. Enter in “Remarks” block of SSA-6562 “First followup sent to FO on xx /xx/xx.”

  6. Refile folder with SSA-6562 for future pulling when diary comes due.

  7. Update control system per RM 05000ff.

B. Second Followup (Pull Date Shown in Block 2 of SSA-6562 Comes Due)

  1. Pull out the second copy of the SSA-5533 U4 in file. It is not necessary to attach file material.

  2. Across the top of the form, in red, write “Attention District Manager, Second Request for Status.”

  3. Forward the followup form to the field office involved.

  4. Cross out pull date in block 1 of SSA-6562. Enter a new pull date in block 2 which is 30 calendar days from the date the followup is sent.

  5. Enter in “Remarks” block of SSA-6562 “Second followup sent to FO on xx/xx/xx.”

  6. Refile folder with SSA-6562 for future pulling when diary comes due.

  7. Update control system per RM 05000ff.

C. Third Followup (Pull Date Shown in Block 3 of SSA-6562 Comes Due)

  1. Photocopy the SSA-5533 U4 in file.

  2. Across the top of the form, in red, write “Third Request for Status—FO has Failed to Respond”.

  3. Forward the followup to the Insurance Programs Branch of the Regional Office responsible for the field office involved.

  4. Cross out the pull date in block 3 of SSA-6562. Enter a pull date in block 4 which is 30 calendar days from the date the third followup is sent.

  5. Enter in “Remarks” block of SSA-6562 “Third followup sent to RO on xx/xx/xx.”

  6. Refile folder with SSA-6562 for future pulling when diary comes due. (See E.6. below.)

  7. Update control system per RM 05000ff.

D. More Than One FO Involved

The instructions in A.-C. above apply to each Form SSA-6562 as its pull date comes due.

E. Processing Incoming Material

Incoming material may result from requests for development or requests for status. The only procedural changes involve updating the Form SSA-6562 and followup sequence. Depending on the situation involved, the necessary action would be as follows:

1. DEVELOPMENT COMPLETE

Process as usual.

2. DEVELOPMENT INCOMPLETE

Request FO to redevelop as needed. Rediary the case by adding 60 calendar days to whichever pull date block on Form SSA-6562 is in force. Enter in the “Remarks” block that the request was returned to the FO for additional development on xx/xx/xx.

3. NO RECORD

When the FO has no record of the request we will need to redevelop. Initiate a new request, including a new Form SSA-6562.

4. FIELD OFFICE NEEDS MORE INFORMATION

When the FO requests additional information, provide it as currently called for. Rediary the case by adding 60 calendar days to whichever pull date block on Form SSA-6562 is currently in force. Enter in the “remarks” block that additional information was provided on xx/xx/xx.

5. FIELD OFFICE STILL DEVELOPING

When an FO responds to a request for status by stating it is still developing the case, rediary the SSA-6562. Rediary the pull date block in force by either adding 30 calendar days or entering the anticipated completion date if the FO provided one.

6. NO RESPONSE

When there has been no response to a request for development or status, follow up as appropriate when the pull date comes due. If the final pull date comes due (i.e. the RO has failed to respond), supervisory personnel will follow up with the Insurance Programs Branch of the RO by telephone to determine the status of the case. Further actions will be determined by the RO reply and which of the possible conditions listed above apply.
