RM 10205.065 Applicant for SSN Mails Application to a Non-Servicing Office

If you receive...

Then...

A routine request for an SS-5, Application for a Social Security Card

Mail the SS-5 to the requester with an unfranked return envelope addressed to the servicing office.

A completed SS-5 and all necessary evidence

 

NOTE: Treat an SS-5 completed by the NH but brought to the office by someone else, as a mail-in application. See RM 10205.059, Third Party Brings Form SS-5 into the Field Office (FO).

  • Enter the application and evidence into SSNAP;

  • Document “signature on application” on Verify and Sign screen in SSNAP; and

  • Return evidence to the applicant by mail, using the SSNAP receipt.

  • If SSNAP is unavailable, use form SSA-2880 Returning Documents Submitted w/SS-5, available on the Document Processing System DPS Production. For additional information on form SSA-2880, see RM 00299.070.

A completed or incomplete SS-5 with no evidence, incomplete evidence, or a mandatory in-person interview is required

  • Return the SS-5 and any evidence submitted using the SSA-L669, Request for Evidence in Support of an SSN Application – U.S. Born Applicant or SSA-L670, Request for Evidence in Support of an SSN Application – Foreign-Born Applicant, as appropriate.

  • Enclose an unfranked return envelope addressed to the servicing office, unless a mandatory in-person interview is required.

An incomplete SS-5 with all necessary evidence; mandatory interview is not required

  • When possible, use the telephone to obtain missing information.

  • Otherwise, return the SS-5 to the applicant with the SSA-L669 or SSA-L670, as appropriate, in an unfranked return envelope addressed to the servicing office.
