When requesting birth, marriage or death certificates from Australia, the following
information must be given as part of the request(s):
Full name of individual at birth,
Date and place of birth, and
Full names of both parents, including the mother’s maiden name.
Full names of both parties, and
Date and place of marriage.
Full name of the deceased,
Date and place of death, and
Full names of deceased’s parents, including the mother’s maiden name.
NOTE: When applying by mail, photocopies of the documents of identity and relationship
to the person whose certificate is being requested are required. See the individual
websites for information on what constitutes proof of identity.
