When a claimant contacts a German agency to file a claim for U.S. benefits, the agency
routes forms D/USA 3 (Agreement Between the FRG and the U.S.A. on Social Security)
and D/USA 4 (Agreement Between the FRG and the U.S.A. on Social Security-Liaison-Certification
Form) to the FSP. The German coverage record will be included if it is readily available.
Follow the procedures below which explain how the FSP should process referrals from
a German agency.
Obtain form SSA-2490-F4 (Part I) and the appropriate RSDI applications and evidence
and forward the claim to DIO with the forms D/USA 3 and D/USA 4 and the German coverage
Destroy the forms D/USA 3 and D/USA 4, unless they contain a request for information
from SSA. In that case, forward the forms to DIO, annotated “already entitled to U.S. benefits.”
