   Potential Entitlement to U.S. Totalization Benefits

   If there is potential entitlement to U.S. totalization benefits, prepare an SSA-e2960-U2-BE to request the Belgian coverage record.

   Claim for Belgian Benefits

   If there is a claim for Belgian benefits, prepare a claims package including:

   • •

     Form SSA-e2960-U2-BE;

   • •

     U.S. totalization earnings record;

   • •

     medical evidence from DIO's files, if the Belgian claim is for disability benefits; and

   • •

     evidence submitted for the Belgian claim.
