Initial claims packages are prepared to request Belgian coverage records and to send information to the Belgian agency.

Follow these steps to prepare a claims package.

   U.S. claim

     Prepare a claims package if a Belgian coverage record is required and has not been requested by the FSP in Brussels.

   • •

     Include Form SSA-e2960-U2-BE in the package.

2. 2. 

   U.S. and Belgian or Belgian claim only

   Prepare a claims package if:

   • •

     the Belgian coverage record is required and has not been requested by the FSP; or

   • •

     the Belgian agency has requested information or documentation.

   Include the following in the claims package:

   • •

     Form SSA-e2960-U2-BE; and

   • •

     information or documentation requested by the Belgian agency.
