GN 01722.006 Requesting Spanish Application Forms from U.S. Residents

A. background

Since the FOs do not stock the application forms required by Spain, OIO must send these forms to the claimants when a claim is filed under the agreement. The form E/USA 2 (Statement of Working Life) provides the information that Spain needs to locate a worker's coverage. The form E/USA is an application for Spanish benefits, and it also provides the work history.

B. Procedure -sending requests

Follow this procedure for requesting Spanish application forms.

1. U.S. Claim

  • Send the claimant a form E/USA 2 with a preaddressed return envelope.

  • Annotate the claims folder to show that form E/USA 2 has been sent to the claimant.

2. U.S. and Spanish or Spanish Claim Only

  • Send the claimant a form E/USA with a preaddressed return envelope.

  • Annotate the claims folder to show that form E/USA has been sent to the claimant.

C. Procedure - controlling requests

Follow this procedure for controlling requests for Spanish application forms.

Step Action
1 Prepare a diary for 60 days.

  • If no response is received, send a second request including a preaddressed return envelope, and

  • Prepare a diary for 30 days.

3

If no response is received after 30 days, route the folder to the International Operations and Totalization Staff (IOTS).
